前列腺电切术后放平下肢方法对患者血压的影响

The impact of putting even legs on blood pressure changes after transurethral resection of prostate

目的:观察采用截石位行经尿道前列腺电切术后患者放平双下肢的不同方法对血压的影响情况。方法:选择60例符合纳入标准的患者,按手术先后顺序随机分为实验组和对照组各30例,实验组术后采用单腿慢放恢复平卧位,对照组采用双腿同时慢放恢复平卧位,记录两组改平卧位前1min及恢复平卧位后1、3、5min的平均压(MAP)。结果:体位改变后实验组血压无明显改变(P>0.05),对照组血压下降明显,与体位改变前比较,差异有显著性意义(P<0.05)。两组患者恢复平卧位后在对应时刻比较,差异有显著性意义(P<0.05)。结论:前列腺电切术后放平双下肢时采用单腿慢放可以减轻患者血压的波动程度,有利于确保围手术期的安全。

Objective：To observe blood pressure after undergoing lithotomy position operation in flat lower limb by different methods.Methods：sixty patients accepted transurethraI resection of prostate（TUPR） were randomized to groups with thirty patients each.After operation,group S were kept single leg slowly in flat.Group D were kept double legs slowly in flat at the same time.MAP were recorded at before（T1）,at 1 min（T2）,3 min（T3）,and 5 min（T4） after changing to horizontal position.Results：Group S had maintained their blood pressure stable.There were no statistical significant changes in terms of mean arteria1 pressure（MAP）between before and after their body position changes（P0.05）.However,both SBP and MAP of patients in group D decreased significantly.Compared with patients of group S and group D,both SBP and MAP of Group D cases were significantly decreased（P0.05）.Conclusion：The application of keeping single leg slowly in flat can lower blood pressure levels and fluctuations for the patients undergoing lithotomy position.It ensure perioperative safety of patients to a certain degree.
