氨基葡萄糖口服联合透明质酸钠关节腔注射治疗膝骨性关节炎疗效观察

Observation on the therapeutic effect of oral administration of glucosamine combined with intraarticular injection of sodium hyaluronate on knee osteoarthritis

目的观察氨基葡萄糖口服联合透明质酸钠关节腔注射治疗膝骨性关节炙的疗效和安全性。方法将82例膝骨性关节炎患者随机分为两组：对照组40例采用膝关节腔内注射透明质酸钠；观察组采用膝关节腔内注射透明质酸钠＋口服硫酸氨基葡萄糖。两组疗程均为5周。治疗前后分别采用Lyrsholm膝关节评分量表评定膝关节功能状况。结果观察组患者Lyrshohn膝关节各项评分及总分显著高于对照组（P〈0．01）；观察组的显效率及总有效率显著高于对照组（x2=9．3360，4．8308，P〈0．01）；两组均未发现严重不良反应；在停止治疗的12个月后随访，观察组的治愈率显著高于对照组（x2=4．0821，P〈0．05）。结论氨基葡萄糖口服联合透明质酸钠关节腔注射治疗膝骨性关节炎具有起效快和疗效持久的双重优点，优于单纯的透明质酸钠膝关节腔内注射，安全性好，值得临床床推荐。

Objective To observe the therapeutic etfect and safety of oral administration of glu- cosamine combined with intra-articular injection of sodium hyaluronate on knee osteoarthritis. Methods Eighty-two patients with knee osteoarthritis were randomly divided into two groups： the control group （40 patients） were given intra-artieular injection of sodium hyaluronate, the observation group （42 patients） were given intra-articular injection of sodium hyaluronate and oral administration of glucosamine sulfate. The course of treatment was 5 weeks in both groups. The functional status of knee were assessed by Lyr- sholm knee rating scale before and after treatment. Results The Lysholm knee various score and the ag- gregate score of patients in observation group were significantly higher than those in control group （P 〈 0.01 ） ; The marked improvement rate and total effective rate of patients in observation group were signifi- cantly higher than those in control group（x2 =9. 3360, 4. 8308, P 〈 0.01 ） ; The serious adverse reac- tions were not found in the two groups. The cure rate of patients in observation group was significantly higher than that in control group （X2 = 4. 0821, P 〈 0.05 ） at 12 months follow-up after treatment. Con- clusions It has dual advantages of rapid effect and lasting efficacy that oral administration of glu- cosamine combined with intra-articular injection of sodium hyaluronate in the treatment of knee osteoar- thritis, it has a better effect than simple intra-articular injection of sodium hyaluronate, and is safe, so is worthy of recommendation in clinics.
