摘要
目的:研究血尿酸(UA)水平与冠心病(CAD)的发生及其严重程度的关系。方法:根据冠状动脉造影结果将患者251例分为CAD组148例和非CAD组103例,以发生病变的血管支数反映冠心病严重程度,将患者分为A、B、C、D4组,分别对应冠状动脉正常、1支、2支、3支病变,比较各组间尿酸水平。根据尿酸水平按四分位数将患者分为第1~4组。结果:(1)CAD组血尿酸水平高于非CAD组(P〈0.叭),Logistic多因素回归分析显示年龄、天冬氨酸转氨酶、尿酸水平为冠心病的危险因素。(2)尿酸水平与冠脉病变支数呈正相关(rs=0.229,P〈0.01),其中女性组尿酸水平与冠脉病变支数呈正相关(rs=0-317,P〈0.01),而男性组无此相关性(rs=-0.36,P=0.671)。(3)血尿酸水平第3、第4四分位的CAD患病率明显高于第1、第2四分位(P〈0.01)。结论:血清尿酸水平与冠心病的患病率有关,且与冠状动脉病变程度旱正相关。
Objective: To investigate the relationship between the serum uric acid level, incidence of coronary artery disease (CAD) and its severity. Methods: A total of 251 patients who underwent coronary angiography in our hospital were included in this study according to the inclusion criteria. Patients were divided into CAD group ( n=148 ) and Non-CAD group (n=103) according to the results of angiography. The severity of CAD was measured in terms of the number of vessels with lesions. Patients were divided into four groups including A (no vessel with lesion), B (one-vessel with lesion), C (two-vessel with lesion) and D (three-vessel with lesion). The serum uric acid levels were compared between groups. Patients were divid- ed into four subgroups according to the serum uric acid levels. Results: (1)The serum uric acid level was higher in CAD pa- tients than that of Non-CAD patients (P 〈 0.01). Logistic regression analysis showed that the age, aspartate aminotransferase (AST) and the serum uric acid level were the independent risk factors of CAD. (2)There was a positive correlation between the serum uric acid level and the severity of CAD(rs = 0.229, P 〈 0.01).The same result was found in female patients (rs = 0.317, P 〈0.01), but there was no such significant difference in male patients (rs = -0.36, P = 0.671).(3)A significantly higher percentage of CAD patients was found in serum uric acid levels in the range of quartile 3 and 4 than that of serum uric acid levels of quartile 1 and 2 (P 〈 0.01). Conclusion: The serum uric acid level was associated with the prevalence rate of CAD, and has a positive correlation with the severity of CAD.
