国产冻干麻疹-流行性腮腺炎联合减毒活疫苗免疫效果研究

Immunological Effect of Domestic Measles and Mumps Combined Attenuated Live Vaccine(Freeze-dried)

目的对市售的冻干麻疹-流行性腮腺炎联合减毒活疫苗(Measles and Mumps Combined Attenuated LiveVaccine,Freeze-dried;MMV-Fd)的免疫效果进行监测和评价。方法用随机整群抽样方法,抽取永兴、永顺县5070名3～49岁儿童,其中2474名儿童接种MMV-Fd(观察组),2596名接种冻干水痘减毒活疫苗(对照组),观察90名儿童免疫前、后血清抗体水平及两年的流行病学效果,采用统计产品与服务解决方案(Statistical Productand Service Solutions SPSS)13.0进行统计学分析。结果 MMV-Fd对流行性腮腺炎的保护率为92.28%;免疫前抗体阴性组(<1﹕2)阳转率为85.71%,阳性组(≥1﹕2)抗体≥4倍增长率为76.81%,差异有统计学意义(2=79.014,P<0.01);免疫后抗体几何平均滴度阳性组(≥1﹕2)明显高于阴性组(<1﹕2),差异有统汁学意义(t=6.117,P<0.01)。结论 MMV-Fd对流行性腮腺炎有较好的免疫效果。

ObjectiVe To make surveillance and assessment on immunological effect of domestic measles and mumps combined attenuated live vaccine （freeze-dried） （MMV-Fd）. Method Random cluster sampling was applied to a number of 5,070 children aged 3-9 years in Yongxin and Yongshun county of hunan province, Among them, 2,474 was immunized with MMV-Fd （observation group）, and the other 2,596 children administered with varicella attenuated live vaccine （freeze-dried）（control group）.The serum antibiotic level of 90 children before and after vaccination, epidemiological effect in later 2 years was studied as well. Adopt statistical product and services solutions 13.0 to make statistical analysis. Result The protection rate of MMV- Fd against epidemiological mumps was 92.28 % ;the conversion rate for the group with negative antibiotic level before vaccination （ 〈 1： 2） is 85.71%, for the positive group （ ≥1： 2）, ≥4 times increase of antibody is 76.81%, the difference has statistical significance （X^2=79.014, P〈0.01 ）.The level of geometric mean titer （GMT）in positive group （ ≥ 1 ： 2） is obviously higher than negative group （ 〈 1： 2）, the difference had statistical significance （t=6.117, P〈0.01 ）. Conclusion MMV-Fd had a good immunological effect against epidemiological mumps.
