AO异形股骨髁支持钢板治疗C型髁部骨折的临床应用(附26例报告)

摘要 目的 介绍应用AO异形股骨髁支持钢板治疗股骨髁部粉碎性骨折 (C型 )的经验 ,并对治疗结果进行评估。方法 采用前外侧切口 ,对于C2 、C3 型骨折一期植骨 ,术后常规对流冲洗 3天 ,3日后行下肢功能康复锻炼器 (CPM )功能锻炼 ,2周后扶拐非负重行走 ,6~ 8周逐渐负重行走。结果  2 6例中 ,优 14例 ,良 7例 ,可 3例 ,差2例 ,优良率为 80 .7% ;随访时间 6~ 2 8个月 (平均 2 0 .5个月 )。结论 AO异形股骨髁支持钢板 ,较易完成复杂髁部骨折的内固定 ,骨折复位满意 ,关节面平整 ,内固定坚强 ,可早期功能锻炼 ,疗效满意 。 Objectives To describe the experience and the results of AO butress Condyle plate in the treatment of femoral condyle fracture ( C classification). Methods twenty-six cases were treated with AO BCP. All the cases that belong to AO classification of C2,C3 were exerted bone grafe in procedure simultaneously applied to irrigation therapy and CPM device, which was applied to the early exercise post-operation. Two weeks post-operation the patient can do un-bearing exercise. Bearing exercise was admitted in 6-8 weeks later after operation. Results After 6 to 28 months (average 20.5 months), follow-up of these 26 cases, 14 were excellent, 7 were good, 3 were general and 2 were bad, the overall excellent rate was 80.7% . Conclusions The AO BCP is well designed, easy to compolish the internal fixation of complex femoral condyle fractures. The correct usage of BCP has the advantages of good reduction, reconseruction of articular surface. Rigid fixation and early exercise. To complex fractures of femoral condyle the fracture especially related to the articular surface, This is a good choice.
