听觉认知电位P_(300)对脑白质疏松症患者认知损害的评定价值 被引量:1

摘要 目的 :探讨不同病因的脑白质疏松症 (leukoaraiosis ,LA)患者听觉认知电位P3 0 0 的不同特征。方法 :选择 15 0例经CT或MRI诊断的LA患者 ,以及 40例正常健康老年人 (作为对照组 )。使用肌电诱发电位仪 ,以“听觉靶 非靶刺激序列”为诱发事件 ,完成P3 0 0 检测。结果 :15 0例LA患者中 ,37例不伴有其它疾病的老年患者较对照组靶和非靶刺激潜伏期N1、P2 、N2 和P3 虽然延长 ,P3 波幅降低 6 .3% ,但其差异均无显著性意义 (P >0 .0 5 ) ;39例伴有脑血管病危险因素的患者较对照组靶和非靶刺激潜伏期N1、P2 与对照组间的差异均无显著性意义 (P >0 .0 5 ) ,靶刺激潜伏期N2 、P3 均明显延长 ,P3 波幅降低 31.3% ;38例伴有脑血管病的LA患者和 36例伴有血管性痴呆的LA患者较对照组靶和非靶刺激潜伏期N1、P2 、N2 和P3 均明显延长 ,P3 波幅分别降低 48.4%和 6 0 .9%。结论 :P3 0 0 能够用于评价LA患者认知功能。 Objective:To investigate the value of auditory P 300 in assessing cognitive dysfunctions of different patients with leukoaraiosis(LA).Method:An oddball paradigm was used to elicit the auditory P 300 from 150 patients diagnosed as having leukoaraiosis according to CT or MRI , 40 normally age matched subjects , by using a Dantec Concerto instrument.Result:Compared with the normal controls, 37 normal aged patients delayed N 1, P 2, N 2 and P 3 latencies elicited by frequent stimuli, and their P 3 amplitude was decreased by 6.3% ; but their had no significant difference( P >0.05). Compared with the normal controls, 39 patients with risk factors of cerebrovascular diseases delayed N 1, P 2 latencies, but their had no significant difference(P>0.05). The patients presented delayed N 2, P 3 latencies elicited by frequent stimuli, and their P 3 amplitude was lowered by 31.3%. 38 patients with cerebrovascular diseases and 36 patients with vascular dementia presented delayed N 1, P 2, N 2 and P 3 latencies elicited by frequent stimuli, and their P 3 amplitude was lowered by 48.4% and 60.9% than compared with that of normal controls.Conclusion:The auditory P 300 could evaluate the cognitive dysfunctions of leukoaraiosis.
