支气管袖状肺叶切除术治疗中央型肺癌疗效观察

Observation of therapeutic effects of bronchial sleeve lobectomy combined with pneumonectomy on patients with central lung tumors

目的探讨支气管袖状肺叶切除术治疗中央型肺癌的临床疗效。方法选取我院收治的28例支气管袖状肺叶切除术(观察组)和34例全肺切除术(对照组)患者,分析两种术后生存率和肺功能改变情况,并比较其围术期并发症发生率及死亡率的差别。结果观察组术后肺活量、第1秒肺活量和最大通气量明显优于对照组(P<0.05);观察组患者1、3及5年生存率明显高于对照组(P<0.05);观察组并发症发生率和死亡率显著低于对照组(P<0.05)。结论支气管肺叶袖式切除术治疗中央型肺癌能有效改善患者术后肺功能,降低并发症发生率和死亡率,提高生活质量和远期生存率。

Objective To study the clinical efficacy of bronchial sleeve lobectomy on patients with central lung tumors.Methods Examined the survival rate and postoperative pulmonary function of 28 patients undergoing bronchial sleeve lobectomy（observed group） and 34 patients undergoing pneumonectomy（control group）.Complication and perioperative mortality were also analyzed.Results The postoperative pulmonary function（VC%,PEV1.0 % and MVV %） of observed group was significantly higher than that of control group（P0.05）;the survival rates of 1,3 and 5-year of observed group were also significantly higher than those of control group（P0.05）;but the rates of complications and mortality of observed group were significantly lower than those of the control group（P0.05）.Conclusion The treatment of central lung tumors with bronchial sleeve lobectomy can improve the postoperative lung function,reduce the rate of complications and mortality,and improve quality of life and long-term survival rate.
