鼻嗜酸性细胞乳头状瘤的临床和病理学特点

Clinicopathological study of nasal oncocytic schneiderian papilloma

目的探讨鼻嗜酸性细胞乳头状瘤的临床及病理组织学特点。方法回顾性分析9例鼻嗜酸性细胞乳头状瘤的临床和病理学特征,并对手术标本进行免疫组织化学染色。结果患者年龄48～66岁,6例发生于上颌窦、2例发生于筛窦、1例发生于鼻腔。镜下见乳头状排列的多层柱状上皮细胞,胞浆内含有嗜酸性颗粒,上皮内含有充满粘液的微囊,有的微囊内聚集多少不等的中性粒细胞形成微脓肿。手术后3例复发,1例恶变。结论鼻嗜酸性细胞乳头状瘤是一种罕见的良性肿瘤,易误诊,术后易复发及恶变,需积极治疗和精心的术后监测。

Objective To investigate the clinical and pathological features of oncocytic schneiderian papilloma. Method The clinical and pathological features of 9 cases of oncocytic schneiderian papilloma were studied by retrospective analysis. Results The age of the patients was ranged from 48 to 66 years. 6 cases occurred in the maxillary sinus,2 cases occurred in the ethmoid sinus, and 1 case occurred in the nasal cavity. The observation under microscope showed papillary arrangement of multi-columnar epithelial cells, containing eosinophilic granular cytoplasm,containing full of mucus and epithelial neutrophil vesicles （micro-abscesses）.In the 3 recurrent cases,1 case had malignant transformation. Conclusions Oncocytic schneiderian papilloma in nasal is a rare benign tumor,with the age of onset of more than 50 years old. It is easy to have recurrence and malignant after operation, therefore it needs aggressive treatment and careful postoperative monitoring.
