温针灸治疗糖尿病性周围神经病变临床观察

Observation of diabetic peripheral neuropathy treated by warm-needling therapy

目的观察温针灸治疗糖尿病周围神经病变(DPN)的疗效。方法将60例DPN患者随机分为2组,治疗组32例予温针灸治疗,对照组28例予西医常规治疗。2组均30 d为1个疗程,2个疗程后检测2组治疗前后神经传导速度、空腹血糖(FPG)、餐后2 h血糖(2 h PG)、总胆固醇(TC)及甘油三酯(TG)变化情况,统计2组主要临床症状、体征改善情况及临床疗效。结果治疗组主要症状、体征改善总有效率均高于对照组(P<0.01)。治疗组治疗后腓总神经传导速度及正中神经传导速度与本组治疗前比较均明显加快(P<0.05),FPG、2 h PG、TC及TG均明显降低(P<0.01),且与对照组治疗后比较差异有统计学意义(P<0.05,P<0.01)。对照组治疗后腓总神经传导速度及正中神经传导速度与本组治疗前比较均明显加快(P<0.05),而FPG、2 h PG、TC及TG无明显变化(P>0.05)。治疗组总有效率87.5%,对照组总有效率35.7%,2组总有效率比较差异有统计学意义(P<0.05),治疗组疗效优于对照组。结论温针灸可明显改善DPN患者临床症状,降低患者血脂和血液黏度,改善微循环。

Objective To observe the therapeutic effect of warm-needling therapy on diabetic peripheral neuropathy（DPN）.Methods 60 patients with DPN were randomly divided into two groups.Treatment group received warm-needling therapy.Control group received routine therapy.The therapeutic course was 30 days I two groups.The changes of nerve conduction velocity,fasting plasma glucose（FPG）,2 h postprandial glucose（2 h PG）,total cholesterol（TC） and triglyceride（TG） were detected after two courses in the two groups before and after treatment.Statistics of the main clinical symptoms and signs and clinical efficacy were done.Results The total effective rate of the main symptoms and signs in treatment group was higher than that in the control group（P0.01）.The peroneal nerve conduction velocity and median nerve conduction velocity after treatment were significantly accelerated in comparison with those before treatment in treatment group（P0.05）,FPG and 2 h PG,TC and TG were significantly lower（P0.01）,there was a significant difference between two groups on FPG and 2 h PG（P0.05,P0.01）.The peroneal nerve conduction velocity and median nerve conduction velocity after treatment were significantly accelerated in comparison with those before treatment in control group（P0.05）,FPG and 2 h PG,TC and TG were not significantly lower（P0.05）.The total effective rate in treatment group（87.5%） was higher than that in control group（35.7%,P0.05）.Conclusion Warm-needling therapy can significantly improve the clinical symptoms of DPN patients,lower blood lipids and blood viscosity,improve microcirculation.
