血小板凝胶治疗Ⅲ～Ⅳ期压疮效果观察

Effects of platelet-rich plasma gel on stage Ⅲ or Ⅳ pressure ulcers

目的初步探讨血小板凝胶治疗Ⅲ-Ⅳ期压疮康复的机制。方法将33例(36处)Ⅲ～Ⅳ期压疮患者随机分为观察组(17处)和对照组(19处),观察组创面及窦道充满的自体富血小板凝胶,创面用无菌透明敷贴封闭,28d后可再次重复;对照组创面给予无菌生理盐水纱布填塞,每天换药。结果两组创面在治疗后3d、7d、14d、28d压疮愈合评分比较,差异有统计学意义(均P<0.01);两组创面治疗后28d创面疗效比较,差异有统计学意义(P<0.01)。结论富血小板凝胶治疗Ⅲ～Ⅳ期压疮有较好疗效。

Objective To explore mechanism of platelet-rich plasma gel applied to the treatment of stage Ⅲ or Ⅳ pressure ulcers. Methods Thirty-three cases with 36 suage Ill or IV pressure ulcers were randomized into two groups. For the observation group （17 pressure ulcers）, autologous platelet-rich plasma gel was applied to the ulcer surface and sinus, and then sterile transparent dres- sing was used to cover the ulcer. The treatment could be repeated after 28 days. For the control group, ulcer surface was stuffed with sterile saline gauze and dressing change was performed everyday. Results The ulcer healing scores had statistically significant differences between the two groups before treatment, at 3, 7, 14 and 28 days after the treatment （P〈0.01 for all） significant difference was found in the curative effect at 28 days after the treatment between the two groups （P〈0.01）. Conclusion Platelet plasma gel can effectively treat stageⅢ or Ⅳ pressure ulcers.
