摘要
本文研究了Dau 4mg/kg iv对11例心律失常患者的心脏电生理的影响,以及不同剂量的Dau(1,3,6,9mg/kg)iv对麻醉犬ECG和BP的影响。临床研究中观察到Dau使PA间期、AH间期和HV间期分别延长15.8,6.1和21.5%;QRS增宽14.3%,HR加快4.1%,而JTc间期无明显改变;使心房ERP和FRP分别延长4.6%和2.5%,房室结不应期无明显改变;对cNRT和SACTc无明显影响。麻醉犬实验中,Dan使PR间期延长,QRS增宽、BP下降,作用与剂量正相关。小剂量HR加快,较大剂量使HR减慢,JTc间期延长。结果表明,Dau的心脏电生理特性与Ic类抗心律失常药物很相似,Dau延缓心脏传导速度,尤其对希蒲系传导的延缓,可能在其抗心律失常效应中起着重要作用。在明显延缓心脏传导同时,对功能正常的窦房结无明显影响。
The electrophysiologic properties of dauricine 4mg/kg given iv were assessed in 11 patients with arrhythmia during intracardiac electrophysiological study. Effects of dauricine on ECG and BP were evalua-ted in 6 anaesthetized dogs. Dauricine increased significantly PA interval (15.8% ), AH interval(6. 1 %) and HV interval (21. 5%), lengthened QRS-width (14. 3%) withJTc being unchanged,prolonged atrial effective and functional refractoryperiod, but did not change A-V nodal refractory period, cSNRT andSACTc. In the anaesthetized dogs, dauricine prolonged PR, QTc and JTcintervals, lengthened QRS-width and lowered BP,these effects were mark-edly dose-dependant. However, heart rate was increased in a dose of1mg/kg and decreased significantly in doses of 6mg/kg and 9mg/kg. The data suggested that dauricine may be classified as an antiarrhyth-mic agent of class Ic type of the Harrison's modification of classification,that its partly important mechanism of antiarrhythmic effect may be rela-ted to slowing couction, specially His-purkinje system conduction, thatdauricine slowed significantly conduction with the function of sinus node being unchanged
