超声对踝关节中心解剖标志的研究 被引量:3

摘要 目的探讨踝关节力学中点的理想的位置,为全膝关节置换术(TKA)下肢力线的准确定位提供好的依据。方法用超声机对64个健康志愿者的踝关节进行测量,保持中立位,量取踝关节平面胫骨前肌腱外侧缘、?母长伸肌腱外侧缘、足背动脉中心、趾长伸肌腱内侧缘和内外踝连线中点至踝关节中心的距离,从而确定最理想踝关节力学中心点的解剖标志,并且在最大外旋位和内旋位时,测量各标志相对中立位时的变化值。结果用超声机在踝关节平面测得的各标志距踝关节的平均距离是:胫骨前肌腱外侧缘(10.2±2.9)mm、拇长伸肌腱外侧缘(2.1±1.3)mm、足背动脉中心(0.5±1.1)mm、趾长伸肌腱内侧缘(-7.1±3.1)mm和内外踝连线中点(-2.3±1.3)mm。足背动脉的中心位于踝关节的中心的内侧,平均距离为(0.5±1.1)mm,与内外踝中心距踝关节中心的平均距离比较差异有统计学意义(P<0.05)。足背动脉与拇长伸肌腱位置较为固定。踝关节最大内旋位时,各体表标志相对中立位时变化较大,而在最大外旋位时各指标变化较小。结论在TKA手术时,足背动脉和拇长伸肌腱外侧缘是最为理想的踝关节力学中心的定位标志。
