彩色M型超声心动图基线移动法评价左室舒张功能观察

摘要 目的 通过二尖瓣血流频谱法结合彩色M型超声心动图基线移动法 ,旨在探讨更准确而简便的左室舒张功能评价手段。方法 冠心病组 74例 ,按左室射血分数和E/A分为 3组 :EF >5 0 %和E/A <1为A组 ;EF >5 0 %和E/A≥ 1为B组 ;EF≤ 5 0 %为C组。另设对照组 2 2例 ,在心尖四腔心切面测量E峰和A峰 ,并计算E/A比值、E峰减速时间 (DT)、等容舒张时间 (IVRT) ,彩色M型多普勒测量血流播散速率 (RFP) ,计算RFP/E。结果 冠心病组的 3个亚组与对照组比较 ,RFP和RFP/E均有显著差异 ;B组或C组的上述指标与A组亦有显著差异 ;B组和C组间无显著差别。结论 彩色M型超声心动图基线移动法能鉴别二尖瓣血流频谱的伪正常 ,能准确评价左心室舒张功能。 Objective The study was seeking for more exact and convenient way of detecting left ventricular diastolic function by color m-mode echocardiography by baseline shift technique.Methods 74 patients suffered from coranary artery disease classified by E/A ratio and LVEF as Group A(EF>50%and E/A<1),Group B(EF>50% and E/A≥1) and Group C(EF≤50%).were enrolled 22 healthy persons treated as controlgroup.E wave, A wave,E/A ratio,DT,IVRT,RFP,RFP/E were detected in everyone.Results The values of RFP and RFP/E were significantly different between control group and other three groups with coranary artere disease as well as between Group A and Group B or Group A and Group C(all P<0.01)。There were no difference between Group B and Group C(P>0.05).Conclusion Color m-mode echocardiography by baseline shift technique is capable in distinguish pseudonormalized transmitral flow pattern and evaluate pricisely left ventricular diastolic function.
