高选迷切术治疗十二指肠溃疡穿孔(附12例报告)

Highly seletive vagotomy in the treatment of perforation of duodenal ulcer:A report 12 cases

目的 探讨高选择性迷走神经切断术治疗十二指肠溃疡穿孔的效果。方法 对 12例十二指肠溃疡穿孔 ,采用修补加高选择性迷走神经切断术的方法。结果 本组 12例 ,穿孔时间 4～ 16小时 ,平均 8小时。穿孔直径 0 .3～ 0 .8cm ,平均 0 .5cm。术后随访 3～ 5 6个月 ,平均 11个月。随访期间经上消化道钡餐和胃镜检查 ,均未见溃疡复发。结论 高选择性迷走神经切断术治疗十二指肠溃疡穿孔 ,手术安全 ,效果明显。

Objectives Discussing the effect of highly selective vagotomy (HSV) and repair treatment in perforation of duodenal ulcer.Methods 12 cases with perforation of doedenal ulcer were treated by the repair and HSV.Results The mean time was 8h(range 4～16)in 12 cases with perforation of duodenal ulcer.The mean focus of diameter was 0.5cm(range 0.3～0.8).The mean postoperative follow-up was 11 months(rangs 3～56).All 12 patients were no recurrent ulcer by gastrophotography and gastroscope.Conclusions HSV is safe and effective method for treament of the perforation of duodenal ulcer.
