小剂量肝素和低分子肝素治疗恶性血液病血栓前状态效果观察

Effect of hematologic malignancies prothrombotic state treated with low-dose Heparin and low Molecular weight heparin

目的对比分析小剂量肝素和低分子肝素对恶性血液病血栓前状态的治疗效果。方法将2007年6月至2011年5月在我院住院初诊并治疗的恶性血液病患者共57例,随机平分为3组实施治疗和观察:单纯化疗组、加用小剂量肝素组、加用低分子肝素组。另从健康体检者选取21例身体健康者作为对照组进行对比研究。结果恶性血液病患者D-二聚体和血浆粘度远高于正常人,差异具有显著性(P<0.01)。低分子组D-二聚体改善程度明显高于小剂量组和化疗组(P<0.05),血浆粘度改善程度明显高于单纯化疗组(P<0.05),小剂量组D-二聚体和血浆粘度改善程度明显高于单纯化疗组(P<0.05)。结论低分子肝素的抗凝作用较小剂量肝素强,且出血倾向减轻。

OBJECTIVE To compare and analyze the effects of hematologic malignancies prothrombotic state treated with low-dose heparin and low molecular weight heparin.METHODS 57 cases of the hematologic malignancies prothrombotic patients newly diagnosed and treated in our hospital from June 2007 to May 2011 were divided into three groups randomly.These three groups were conducted treatment and observation,named chemotherapy alone group,plus low-dose heparin group,plus low molecular weight heparin group.In addition,21 cases of healthy volunteers were selected from physical examination people were treated as control group.RESULTS Patients with hematologic malignancies D-dimer and plasma viscosity were much higher than normal ones,the difference was significant（P0.01）.In the plus low molecular weight heparin group,D-dimer had improved better than the groups chemotherapy alone and plus low-dose heparin（P0.05）,plasma viscosity better than chemotherapy alone group（P0.05）.In the plus low-dose heparin group,D-dimer and plasma viscosity had improved better than chemotherapy alone group（P0.05）.CONCLUSION The anticoagulant effect of the low molecular weight heparin low-dose heparin is strong,and it can reduce the bleeding tendency.
