WONCA研究论文摘要汇编(七十五)——空气污染小时差异及其对心肌梗死风险的作用:英国心肌梗死项目基础(MINAP)数据库病例交叉分析 被引量:1

摘要 目的用小时瞬时清晰度数据,就短时间量程空气污染与心肌梗死相关性进行调查。设计根据英国空气质量对悬浮微粒(PM10)、臭氧(ozone)、一氧化碳(CO)、二氧化氮(NO2)和二氧化硫(SO2)测量记录,英国心肌梗死项目临床相关数据时间层病例交叉研究。经在月历层内,对周围温度、相对湿度、流感和呼吸合胞病毒循环水平、周历、假日及剩余季节特性校正后,在单及多元污染物模型中,调查1~6、7~12、13~18、19~24和25~72 h污染延迟作用。场所英格兰及威尔士15个卫星城市人群基础研究。受试者2003—2006年,79 288例诊断为心肌梗死病人。主要结果测量污染水平每增加10μg/m3所带来的心肌梗死剩余风险。结果单污染物模式,PM10和NO2水平与极短期1~6 h后心肌梗死风险增加相关{每增加10μg/m3,剩余风险分别为1.2%[95%CI(0.3,2.1)]和1.1%[95%CI(0.3,1.8)]};多元污染物模型,这种作用依然持续着,尽管仅获得了独立PM10作用的弱证据(P=0.05)。即刻风险增加,并随着时间段延长而减少:我们未发现暴露72 h后,任何净剩余风险与5种污染物研究相关证据。结论与交通工具相关污染典型标识物,即PM10和NO2水平高,似乎与暴露1~6 h后短暂心肌梗死相关。但之后风险减少提示空气污染可能与促发某时间点事件相关(短期置换)而与总风险增加无关。可确定,空气污染对心肺死亡率的影响,不是经心肌梗死急性风险增加而是另有机制所致。
