单纯吻合指动脉行末节断指再植疗效观察

Therapeutic effects on paratelum severed finger reunion in simple artery anastomosis

目的探讨单纯吻合指动脉末节断指再植的方法和疗效。方法回顾性分析单纯吻合指动脉的末节断指再植34例(39指),按照常规方法行指动脉端端吻合,所有病例均行甲床放血,部分病例行再植指侧方切开放血,维持3～5d即可建立有效循环。结果 39指成活36指,失败3指,成活率92.3%。术后随访3～6个月,再植指体指腹饱满,皮色及皮温正常,再植指体远端痛温觉均有不同程度恢复,两点辨别觉为3～6mm。结论单纯吻合指动脉的末节断指再植,缩短手术时间,提高成活率,术后外形及功能恢复良好。

Objective To investigate the methods and therapeutic effects on paratelum severed finger reunion in simple artery anastomosis. Methods The paratelum severed finger reunion in simple artery anastomosis of 39 fingers in 34 cases was retrospectively analyzed. The end-to-end anastomosis of arteria digitalis was performed in accordance with conventional method. Hyponychium blood- letting was performed in all cases, bloodletting by latero-incision was performed in some cases. Effective blood circulation can be estab- lished after 3-5 days of bloodletting. Results Thirty-six fingers survived, 3 failed, the survivial rate being 92.3%. Postoperative fol- low-up was performed for 3-6 months. The finger pulp of reimplantation finger was well-stacked, whose color and temperature were nor~ mal. The pain thalposis at the distal end of reimplantation finger got recovered in some degree. The epicritic sensibility between two points was 3-6 mm. Conclusion Simple artery anastomosis of finger distal segment replantation shortens the operation time and raises survival rate, resulting in satisfactory recovery of the shape and function of the replanted fingers.
