摘要
目的研究首发抑郁障碍患者血清IL-17治疗前后变化,及治疗前血清IL-17与HAMD、LES相关量表的关系,进一步明确免疫状态的改变与抑郁障碍病因与发病机制之间的关系。方法病例组40例为首发抑郁障碍患者于SSRI类药物治疗前后分别采用酶联免疫吸附法(ELSIA)测定血清IL-17水平,并且评定汉密尔顿抑郁量表(Hamilton depression rating scale for depression,HAMD)及生活事件量表(Life event scale,LES),与40例对照组比较。结果 (1)首发抑郁障碍患者治疗前血清IL-17水平(41.28±3.70)高于对照组(21.68±3.72)和治疗后(30.67±3.47),差别具有统计学意义(P<0.05);首发抑郁障碍患者SSRI类药物治疗后血清IL-17水平(30.67±3.70)高于对照组(21.68±3.72),差别具有统计学意义(P<0.05)。(2)治疗前HAMD评分(20.73±3.13)显著高于治疗后(6.78±4.30),具有统计学意义(P<0.05)。(3)正常人群血清IL-17浓度与年龄呈正相关(r=0.57,P<0.05);首发抑郁障碍患者治疗前后血清IL-17浓度与年龄均无相关(P>0.05),与治疗前后HAMD评分呈正相关(r=0.61,P<0.05)(r=0.68,P<0.05);首发抑郁障碍患者治疗前血清IL-17水平与LES负性分值、总分成正相关(r=0.4,P<0.05;r=0.39,P<0.05),与LES正性分值无相关(P>0.05)。结论首发抑郁障碍患者血清IL-17水平存在异常,且在治疗前后存在变化;首发抑郁障碍患者严重程度、所经历的负性生活事件与血清IL-17存在一定关联性;首发抑郁障碍存在自身免疫功能紊乱。
Objective To study the variation of serum IL-17 of First depressive disorder patients before and after treatment and relationship with HAMD and LES. In order to clear the relationship between changes of IL-17 in immune status and the etiology of depressive disorder and pathogenesis. Methods In the study, 40 First depressive disorder patients, who were cured before and after by SSRI drugs, and 40 healthy control subjects were determined the serum IL-17, using ELSIA, and assessed using HAMD and LES. Results (1) The First depressive disorder patients, before treatment, showed increased scrum level of IL-17 compared with the healthy control group(21.68±3.72) and the case group after treatment(30.67±3.47), P0.05,Significantly. The case group after treatment showed increased scrum IL-17 compared with the healthy control group, P0.05., Significantly. (2) In the case group, HAMD scores, before treatment,(20.73±3.13) showed Significantly higher than after treatment(6.78±4.30),P0.05., Significantly. (3) In the control group, Serum concentration of IL-17 and age was positively correlated (r=0.57,P0.05); In the case group, before and after treatment, Serum concentration of IL-17 and age were not correlated(P0.05), but was positively correlated (r=0.68,P0.05) with HAMD scores.; The case group, before treatment, Serum concentration of IL-17 and LES negative score (r=0.4, P0.05)、LES score(r=0.39,P0.05) were positively correlated, but no correlation with LES positive score(P0.05). Conclusion Among the First depressive disorder patients, the level of serum IL-17 was abnormal, and varied before and after treatment. There is a certain correlation between the severity of the disease、Experienced negative life events and the level of serum IL-17. First depressive disorder exists autoimmune disorders.
