经皮椎体成形术治疗老年骨质疏松性脊柱骨折临床分析

Treating osteoporotic vertebral fracture with percutaneous vertebroplasty in elderly patients

目的:探讨椎体成形术治疗老年骨质疏松性脊柱骨折的方法和疗效。方法:将100例确诊的老年骨质疏松脊柱骨折患者随机分为观察组和对照组,观察组行经皮椎体成形术(PVP),对照组行传统保守治疗,比较两组患者疗效、影像学变化情况和并发症发生情况。结果:观察组和对照组治疗后VAS评分、Oswestry评分均较术前显著下降(P均<0.05),观察组下降幅度显著大于对照组(P<0.05)。观察组治疗后伤椎后凸角、伤椎前缘高度、伤椎中线高度及伤椎后缘高度均较治疗前显著提高(P均<0.05),对照组治疗后以上影像学指标均与治疗前无统计学差异(P>0.05)。观察组、对照组治疗后Barthel指数均较治疗前显著提高(P均<0.05),观察组Barthel指数恢复显著优于对照组(P<0.05)。结论:PVP为治疗老年骨质疏松脊柱骨折的有效手段,能更好改善症状,恢复椎体形态,提高患者生活质量。

Objective： To investigate the treatment effects of percutaneous vertebroplasty on osteoporotic vertebral fracture.Methods： A total of 100 elder patients with osteoporotic vertebral fracture were randomly divided into observation group and control group with 50 cases in each group.The observation group was treated with percutaneous vertebroplasty（PVP）,while the control group was treated with conventional treatment.Treatment effects,imaging changes and incidence of complications were observed and compared.Results： After treatment,VAS score,Oswestry score of observation group and control group both significantly decreased significantly compared with preoperative status（P0.05）,but the decrease in the observation group was more obvious（P0.05）.After treatment the vertebral kyphosis,front height,midline height and trailing edge height of the involved vertebral were all significantly improved for the observation group（P0.05）.However,the no significant difference in the above mentioned parameters were observed for the control group（P0.05）.Both groups showed increased Barthel index after treatment（P both0.05）,but the improvement was more obvious in the observation group（P0.05）.Conclusion： PVP treatment is an effective treatment for osteoporotic vertebral fracture in elderly patients since it can improve symptoms,help to restore vertebral body shape and raise patients＇life quality.
