产前护理干预对先天性心脏病孕妇分娩结局的影响

Application of prenatal nursing in women delivery with congenital heart disease

目的探讨产前护理在先天性心脏病（简称先心病）孕妇分娩中的应用效果。方法选择2003--2010年在我院妇产科住院进行分娩的先心病孕妇33例作为研究组，实施心理护理、合理膳食、适量活动、适量吸氧等护理措施。按抽签法选择同期33例相同年龄段的孕妇作为对照组，比较两组孕妇在分娩方式、产程时间、分娩过程中并发症、胎儿评分等方面的差异。结果研究组均顺利分娩，其产程时间为（3．45±4．42）h，与对照组（3．86±3．89）h比较，差异无统计学意义（t=-0．931，P〉0．05）；研究组胎儿Apgar评分为（8．82±0．68）分，对照组为（8．91±0．68）分，两组比较差异无统计学意义（t=-1．048，P〉0．05）两组孕妇在分娩方式、产程中并发症、住院时间方面比较，差异无统计学意义（P〉0．05）。结论良好的产前护理对先心病孕妇非常重要，经过精心的产前护理后，先心病孕妇分娩风险接近普通孕妇水平。

Objective To study the effect of prenatal nursing in the safety childbirth among pregnant women with congenital heart disease. Methods Totals of 33 pregnant women in experimental group with congenital heart disease in 2003 to 2010 and 33 pregnant women in the same period were randomly selected to be control group. Experimental group were received prenatal nursing, rational diet , moderate physical activities , moderate oxygen and so on ,then ,the delivery way, duration of delivery, complications during delivery, Apgar score and average in-hospital days were evaluated and compared in two groups. Results Experimental group pregnant were successfully laboured , and the average duration of delivery was （ 3.45 ± 4.42 ） hours and （ 3.86 ± 3.89 ） hours of control group, the difference was no statistically significant （ t =- - 0. 931, P 〉 0. 05 ）. The average Apgar score was（ 8.82 ± 0.68 ） in experimental group and （ 8.91 ± 0.68 ） in control group , and difference was no statistically significant （ t = - 1. 048, P 〉 0. 05 ）. There was no significantly difference in delivery way, complications during delivery, average in-hospital days in two groups （ P 〉 0. 05 ）. Condusions Carefully prenatal nursing is very important for those pregnant women with congenital heart disease, which can promote these pregnant to achieve a safe delivery as the normal pregnant women.
