不合理处方的分析报告

Analysis of Unreasonable Prescription

目的了解门诊不合理用药处方情况,分析原因,提出解决不合理用药措施。方法随机抽取我院2010年7月至2011年6月门诊处方6000张(每月随机抽取500张,共计6000张),根据药品说明书,临床药理学知识及文献资料,对不合理处方进行分类、分析。结果不合理处方主要表现在处方的书写不规范,及临床用药的不合理。其中不合理用药包括药物配伍不合理、药物用法用量不当、2个科室重复用药、给药方案、无明确指征,跨科室用药等。结论故针对以上表现形式,有必要对我院的处方进行系统的、全方位的思考,采取多方面的、科学的干预措施,才能切实有效的提高我院合理用药的水平。

Objective Understand the situation of unreasonable drug usage prescription at out-patient department and analyze the reasons,put forward the measures to solve this problem.Methods Firstly,we should pick 6000 prescriptions at random from July 2010 to June 2011（pick 500 prescriptions at random monthly,totally 6000 prescriptions）,and then we classify and analyze them Result According to the medical instruction,clinical pharmacology of knowledge and Literature,find out the main phenomenon are that the writing format of prescriptions unregulated and clinical application unreasonable,the latter phenomenon including the compatibility of medicines unreasonable,Improper usage and dosage of the drug,repeat using drug from different sections,the treatments solution without the clear indication and so on.Conclusion Therefore,according to above expressions,we should systematically and completely consider this problem and take various,scientific intervention measures solve it,ensure effectively improve the ability of our hospital’s reasonable drug usage.
