低温射频消融术治疗老年OSAHS效果观察 被引量:1

摘要 目的观察局麻下低温射频消融术(TCRF)治疗老年阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的疗效。方法老年OSAHS患者33例,局麻下行TCRF治疗。术前及术后6个月分别进行多导睡眠监测(PSG),采用Epworth嗜睡量表(ESS)评价患者白天嗜睡情况,分析比较手术前后患者睡眠结构的变化。结果 33例患者均顺利完成手术及全程监测。均未出现大出血、感染、吞咽困难、窒息等严重并发症。术后6个月复查PSG,呼吸暂停低通气指数(AHI)为(13.9±7.5)次/h,最低血氧饱和度(LSaO2)为82.3%±4.9%,微觉醒指数为(33.2±11.2)次/h,睡眠效率为81.5%±10.2%,非快速眼动(NREM)3+4期所占睡眠时间百分比为5.2%±4.7%,快速眼动(REM)期所占睡眠时间百分比为16.4%±4.1%,ESS总分为(6.7±3.6)分,均较术前明显改善(P均<0.05)。根据OSAHS疗效评定标准,治愈8例、显效12例、有效9例、无效4例,总有效率为87.9%。结论 TCRF治疗老年OSAHS可改善AHI、LSaO2,患者微觉醒指数降低,睡眠效率提高,REM期和NREM3+4期占睡眠时间百分比提高,疗效较好。
