摘要
目的 探讨最佳肺泡内氧浓度及温度对肺缺血再灌注损伤的保护作用。 方法 离体兔肺在不同肺泡内氧浓度及不同温度下用非氧合血行肺动脉灌注 30分钟 ,测定支气管肺泡灌洗液中血浆白蛋白 (AL B) ,肺表面活性蛋白 (SP,SP- A,SP- B,SP- C)占总蛋白含量的百分比 ,PS的活性并进行组织学观察。 结果 肺泡内氧浓度与 SP含量及活性成正比 ,与 AL B含量成反比 (P<0 .0 1) ;肺泡内温度与 AL B含量成正比 ,与 SP含量及活性成反比 (P<0 .0 1)。结论 (1)离体兔肺损伤明显 ,并减低了 SP的含量和活性 ;(2 )支气管肺泡灌洗液中 SP,AL B含量和 PS活性的测定是评价离体肺缺血再灌注损伤程度的可靠指标 ;(3)肺泡内充氧和低温对离体兔肺 SP合成、活性、肺泡 型细胞结构和功能有较好的保护作用。
Objective To investigate the protective effect of the best intra alveolar oxygen concentration and temperature on the ischemia reperfusion lung injury. Methods The pulmonary artery of donor lung was perfused with no oxygen blood for 30minutes under different temperature and intra alveolar oxygen concentration, the ratio of albumin (ALB) and surfactant protein (SP, SP A, SP B, SP C) to the total protein in the bronchoalveolar lavage fluid was detected. Results There were direct ratio relationship between concentration and activity of the SP A, SP B, SP C and oxygen concentration ( P <0 01), and inverse ratio relationship between content of ALB and oxygen concentration ( P <0 01). There were inverse ratio relationship between the SP content and activity of the SP and temperature ( P <0 01), and the direct ratio relationship between content of ALB and temperature. Conclusion (1) The ischemia reperfusion injury to isolation lung reduces significantly the content and activity of SP. (2) The concentration of SP, ALB, and activity of SP in bronchoalvelolar lavage fluid are the reliable index that is used to evaluate the degree of ischemia reperfusion lung injury following isolated lung. (3) The higher intra alveolar O 2 concentration and low temperature have the protective effects on metabolism syntheses and activity of SP and the structure as well as the function of type Ⅱ cell following isolated lung.
