Hysteroscopic Lysis of intrauterine Adhesions By Nd∶YAG Laser

目的 探讨宫腔镜下Nd∶YAG激光手术治疗宫腔粘连的安全性和有效性。方法 对 1994年 5月至 1999年 6月期间施宫腔镜下Nd∶YAG激光手术治疗宫腔肌性或纤维性粘连的 5 3例患者的临床资料进行总结分析。结果 术后 5 3例患者宫腔均恢复正常形态 ,其中轻中度粘连患者共 42例均行 1次手术 ,重度粘连患者 11例中 3例行 1次手术 ,7例行 2次手术 ,1例行 3次手术。术后月经恢复正常率为 76 .5 % (2 6 / 34 ) ,月经恢复有效率为 10 0 % (34 / 34 ) ;术后妊娠率为6 2 .5 % (10 / 16 )。无术中、术后并发症。结论 宫腔镜下Nd∶YAG激光治疗宫腔粘连是一种安全、可行和有效的方法。

Objective To evaluate the effectiveness and safety of hysteroscopic Nd∶YAG laser treatment of intrauterine adhesions. Methods 53 cases of intrauterine muscular and fibrous adhesions were treated using Nd∶YAG laser cutting and vaporization from May, 1994 to June, 1999. Results After operation, all of the 53 cases uterus cavity recovered to normal shape. 42 cases of them were mild and intermediate grade, who experienced operation only once. 11 cases were severe grade, 3 of them experienced operation once, 7 of them experienced twice, one case experienced three times. The normal menstruation recovery rate was 76 5% (26/34) and effective rate 100%(34/34). The pregnancy rate was 62 5%(10/16). No intraoperative or postoperative complication was found. Conclusions We would conclude that treatment of intrauterine adhesions with hysteroscopic Nd∶YAG laser irradiation is a safe, feasible and highly effective new procedure for the treatment of intrauterine adhesions.
