多发伤患者腹内压增高对肝肾功能的影响

Effects of high intra-abdominal pressure on hepatorenal function of multi-trauma patients

目的探讨严重多发伤患者腹内压增高对肝肾功能的影响。方法从2007年1～6月收治的多发伤患者中,选择40例多发伤患者按照创伤评分分为重伤组和轻伤组,进行腹内压监测,并收集患者肝肾功能等资料进行分析。结果与重伤组相比较,轻伤组患者腹内压明显降低,ALT降低,尿量增多,差异具有统计学意义(P<0.01);血清TBIL、Cr、BUN两组差异无统计学意义。结论腹内压升高可致肝肾功能下降,这将影响患者的康复,早期及时的控制腹内压升高可能降低肝肾功能损伤。

Objective To investigate the effects of high intra-abdominal pressure（IAP） on hepatorenal function of multi-trauma patients.Methods According to injury severity score（ISS）,forty multi-trauma patients were divided into two groups,the severe multi-trauma group（ISS≥16） and the mild multi-trauma group（ISS16）.The data of IAP and hepatorenal function were analyzed.Results The levels of IAP were in the severe multi-trauma group were significantly higher than those in the mild multi-trauma group,and the incidence of impaired hepatorenal functions in the severe multi-trauma group were significantly higher.Conclusion The hepatorenal function may be damaged when the levels of IAP are on rise.Control IAP in time at early stage could benefit severe multi-trauma patients by reducing hepatorenal impairment.
