包卷式胰肠吻合术在腹腔镜胰十二指肠切除的临床价值

Clinical value of embedding style of pancreaticoenterostomy in laparoscopic pancreatoduodenectomy

目的探讨包卷式胰肠吻合术在腹腔镜胰十二指肠切除术(LPD)预防胰漏的临床价值。方法回顾2002年11月至2009年5月6年间26例行LPD术中术后的情况。结果 26例LPD中行22例包卷式胰肠吻合术式的手术时间(490.7±77.8)(382～660)min,胃肠功能恢复时间(4.2±1.3)d,胰漏发生率9.1%,术后并发症发生率45.5%,住院时间(29.3±14.5)d,无死亡病例。结论包卷式胰肠吻合术在作为LPD术中胰肠吻合方式是一种积极的可靠的安全的外科治疗术式,具有重要的临床应用价值。

【Objective】To evaluate embedding style of pancreatico-enterostomy prevention of pancreatic fistula after Laparoscopic pancreatoduodenectomy（LPD）.【Methods】26 patients with LPD between November 2002 and May 2009 were reviewed.【Results】26 patients with LPD were included in the study.The 22 patients underwent embedding style of pancreaticoenterostomy,operation time（490.7±77.8）（382～660） min,functional rehabilitation of gastrointestinal tract time（4.2 ±1.3） d,pancreatic fistula incidence rate 9.1%,postoperative complications incidence rate 45.5%,length of stay（29.3 ±14.5） d.【Conclusions】Laparoscopic embedding style of pancreaticoenterostomy could prevent pancreatic fistula after LPD,as all aggressive and dependable surgical approach,offers hope for these patients.
