MSCT多期扫描对肾嗜酸细胞腺瘤与嫌色细胞癌的鉴别诊断价值

Differentiation of Oncocytoma and Renal Cell Carcinoma: The Role of 4 Phase MSCT

目的探讨多层螺旋CT(MSCT)四期扫描对肾嗜酸细胞腺瘤(renal oncocytoma,RO)与嫌色细胞癌(chro-mophobe cell renal carcinoma,CCRC)的鉴别诊断价值。资料与方法搜集经手术病理证实的8例(9个)RO和15例CCRC患者的资料,术前全部行CT平扫和三期增强扫描,采用SPSS 16.0软件对两种肿瘤的影像学表现进行统计学处理。结果病变平扫时密度值、钙化、强化的均匀性及中央星状瘢痕在RO组和CCRC组间的差异具有统计学意义(P<0.05);病变的大小、增强扫描各期的强化程度以及病变出现包膜、出血、坏死囊变、"辐轮"状强化及节段性增强反转等征象的比较,差异无显著性统计学意义(P>0.05)。结论 MSCT对术前RO和CCRC的鉴别诊断具有重要价值,平扫密度值、钙化、强化的均匀性及中央星状瘢痕有助于两者的鉴别。

Objective To investigate the use of 4 phase multi-slice spiral CT（MSCT） to help distinguish renal oncocytoma from chromophobe cell renal carcinoma（CCRC）.Materials and Methods Eight patients with 9 oncocytomas and 15 cases of CCRC proved surgically and pathologically were performed in plain and enhanced CT scans.The MSCT findings were analyzed and dealed by SPSS 16.0 software.Results There were statistically significant differences between oncocy-toma and CCRC in pre contrast CT attenuation value,calcification,enhancement homogeneity and central scar（P〈0.05）.The average size,degrees of enhancement in individual contrast phases,capsule,hemorrhage,necrosis cyst,Spoken wheel like enhancement and segmental enhancement inversion had no statistical significant differences between oncocytoma and CCRC（P〈0.05）.Conclusion The MSCT have important value in preoperative distinguishion between renal oncocytoma and CCRC.The pre contrast CT attenuation value,calcification,enhancement homogeneity and central scar can contribute to the differentiation of the 2 renal neoplasms.
