耳穴穴注缓解癌痛对患者生活质量影响的临床研究

Clinical Analysis on QOL of Ear Acupoints Injection Treating Cancer Pain Patients

目的:观察耳穴穴注野木瓜注射液(IS)治疗中重度癌痛对患者生活质量的影响。方法:临床选取179例中重度癌痛患者,随机分为治疗组88例(耳穴穴注IS配合口服奥施康定)和对照组91例(口服奥施康定),7天为1个疗程。分别观察两组的镇痛疗效及对患者生活质量的影响。结果:①NRS评分:治疗前治疗组为(8.27±1.577),对照组为(8.24±1.493),治疗后治疗组为(1.63±1.223),对照组为(2.21±1.185),两组治疗前后比较差异均有统计学意义(P<0.01),治疗后两组比较差异有统计学意义(P<0.05)。②KPS评分:治疗组治疗前为(64.40±5.991),治疗后为(73.22±6.613),对照组治疗前为(64.78±6.009),治疗后为(73.17±6.814),两组前后比较差异均有统计学意义(P<0.01),治疗后两组间比较差异无统计学意义(P>0.05);③BPQ调查显示两组治疗后在日常生活、情绪、行走能力、睡眠、生活乐趣和QOL的总积分上均较治疗前明显改善,前后比较差异有统计学意义(P<0.05);同时治疗后两组间比较治疗组在日常生活、情绪、睡眠、生活乐趣和QOL的总积分上较对照组改善更加明显,两组比较差异有统计学意义(P<0.05)。结论:耳穴穴注IS缓解癌痛有较好的临床疗效,能提高癌痛患者的生活质量。

Objective：To study the QOL in the moderate and severe cancer pain patients with the ear acupoints injection with stauntoniae.Methods：In the clinical study,179 patients with moderate or severe cancer pain were observed,randomly divided into two groups,88 in the treating group were treated by ear acupoints injection with stauntoniae and 91 in the control group took Oxycodon tablets orally to relieve cancer pain,time of the therapy was 7 days.The clinical results to relieve cancer pain and the QOL were compared between the two groups.Results：①Mean NRS of the treating group was 8.27±1.577 before the treatment,1.63±1.223 after the treatment,and in the control group it was 8.24±1.493 before the treatment,2.21±1.185 after the treatment,there were statistical significant differences fore-and-aft in both of the two groups（P0.01）,compared the two groups after treatment,there was statistical difference（P0.05）.②Mean KPS was improved from 64.40±5.991 to 73.22±6.613 in the treating group,and from 64.78±6.009 to 73.17±6.814 in the control group,there were statistical significant differences fore-and-aft in both of the two groups（P0.01）;compared the two groups after treatment,there was no statistical difference（P0.05）.③Both of the two treatments promoted the daily life,emotion,walking ability,sleeping,life joy and total integral through the BPQ investigation,compared with those after treatment,there were statistical differences pro-and-aft（P0.05）;while daily life,emotion,sleeping,life joy and total integral of the treating group were better than those in the control group after treatment,there were statistical significant differences（P0.05）.Conclusion：Ear acupoints injection with stauntoniae was effective to relieve cancer pain,and could improve the QOL better than the Oxycontin.
