摘要
目的 探讨经颅能量多普勒血流显像 (PDI)在脑动静脉畸形 (AVM )诊断中的应用价值。方法对临床诊断为脑动静脉畸形患者 2 5例 ,分别采用经颅能量多普勒和彩色多普勒血流显像 (CDFI)进行探测 ,测量病变血管的Vmax、Vmin及RI ,并与脑血管造影结果进行对比分析。结果 CDFI诊断 2 3例脑AVM ,检出供血动脉 3 7支 ( 80 .4% ) ,主要供血动脉 2 8支 ( 90 .3 % ) ,引流静脉 6支 ( 3 0 .0 % ) ;PDI诊断 2 3例AVM ,检出供血动 42支 ( 91.3 % ) ,主要供血动脉 2 9支 ( 93 .5 % ) ,引流静脉 10支 ( 5 0 .0 % )。PDI和CDFI对AVM大小及位置的判断与脑血管造影结果基本一致 (P >0 .0 5 )。结论 经颅PDI对脑AVM诊断率与CDFI基本一致 ,但在显示非主要供血动脉和引流静脉方面优于CDFI ,可为临床诊断脑AVM提供更丰富的血流动力学资料。
Objective To evaluate potential of transcranial power Doppler imaging(PDI) in the diagnosis of cerebral arteriovenous malformation(AVM).Methods Compared with the results of cerebral angiography.Twenty five cases (16 males, 9 females),of cerebral AVM were involved.Transcranial PDI and color Doppler flow imaging(CDFI) examination were performed through skull acoustic windows. Hemodynamics parameters such as V max ,V min and RI were measured in abnormal vessels. Results Both transcranial PDI and CDFI detected 23 cases of AVM, the size and location of lesions were well correlated to the results obtained by cerebral angiography.The number of AVM feeding arteries , main feeding arteries and draining veins counted by PDI were 42,29,10, respectively,and those by CDFI were 37,28,6, respectively.Conclusions There ware no much difference in the diagnostic accuracy of AVM between transcranial PDI and CDFI. Transcranial PDI shows more advantage than CDFI in displaying atypical AVM feeding arteries and draining veins.
