摘要
目的探讨磁共振弥散加权成像在儿童腹部良恶性肿瘤及恶性肿瘤化疗前后比较的应用价值。方法对50例儿童腹部肿瘤及肿瘤样病变进行磁共振弥散加权成像(diffusion-weighted imaging,DWI)扫描,选择病变组织实质性成分较多的区域(region of interest,ROI)进行表观弥散系数(apparent diffusion coefficient,ADC)测定。对20例腹部恶性肿瘤中于化疗后2~3个疗程或手术化疗后2~3个疗程进行DWI复查。结果①恶性肿瘤36例,平均ADC值为0.000778mm/s2。良性腹部肿瘤14例,平均ADC值为0.00199mm/s2。②20例腹部恶性肿瘤于化疗后随访,体积均明显缩小;DWI结果显示ADC值明显升高。③对两组ADC值进行统计学分析,恶性组与良性组之间有统计学意义。④20例恶性肿瘤化疗前后的ADC值变化进行分析,有统计学意义。结论弥散加权成像技术可以作为肿瘤性质判断以及肿瘤对于化疗药物反应的监测工具。
Objective To discuss the application value of MR diffusion-weighted imaging(DWI) in diagnosing children abdominal benign and malignant tumors and comparing the distinction before and after chemotherapy in malignant tumors.Methods 50 cases of malignant and benign abdominal masses were performed DWI,and the region of interests(ROI) in lesions received the ADC measure.20 cases of malignant tumors were performed DWI again post two to three durations of chemotherapy.Results ①36 cases were malignant tumors,and ADC values were 0.000778 mm/s^2.14 cases were benign tumors,ADC values were 0.00199 mm/s^2.②20 cases of malignant tumors were followed-up after chemotherapy,and the tumors were obviously shrinked with the ADC values increased.③There was a significant difference of the ADC values between malignant tumors group and benign tumors group.④There was a significant difference of the ADC value in pre and post chemotherapy of malignant tumors group.Conclusions DWI provides information for differentiating the malignant and benign masses,and it is also a good modality in showing the reaction of the chemotherapy.
