肺动脉支气管成形重建术治疗期非小细胞肺癌

Bronchovascular reconstruction in patients with stage Ⅲ non small cell lung cancer

目的 :为了提高 期非小细胞肺癌的手术切除率 ,减少全肺切除率并获得与全肺切除相同的手术彻底性。方法 :用肺动脉成形同时行支气管成形术治疗 期非小细胞肺癌 1 9例 ,其中鳞癌1 3例 ,腺癌 6例。左上叶袖状切除 1 1例 ,右上叶 6例 ,右中上叶 2例。结果 :本组无手术死亡 ,术后1例发生支气管胸膜瘘 ,余 1 8例无并发症 ,术后 1年及 3年生存率分别为 83%和 39%。结论 :肺动脉支气管成形重建术的应用最大限度地切除了肿瘤组织 。

Objective To increase the resection rates of the stage Ⅲ non small cell lung cancer,to reduce the total pneumonectomy rates,and to achieve the same effect of redical resection as total pneumonectomy. Methods Nineteen patients with stage Ⅲ non small cell lung cancer underwent bronchovascular reconstruction,13 of 19 patients with epidermoid carcinoma and 6 with adenocarcinoma.Brochovascular reconstructions were performed for left upper lobectomy ( n =11),right upper lobectomy ( n =6),right upper middle lobectomy ( n =2).Results All of the operated patients had no postoperative motality.Only one patient had postoperative bronchopleural fistula,and others had no postoperative complications.The 1 and 3 year survival rates were 83% and 39% respectively.Conclusion Bronchovascular reconstruction resected tumor tissues as radically as possible and retained the pulmonary function as greatly as possible.
