摘要
目的:研究经鼻与经口气管插管在呼吸衰竭患者中的治疗效果的对比观察。方法 :31例呼吸衰竭患者随机分为经鼻气管插管组(14例)、经口气管插管组(17例),分别简称经鼻组和经口组。经鼻组患者经鼻气管插管,经口组患者经口气管插管,观察两组患者治疗前后血气变化、置管时间、鼻窦炎感染情况、口腔护理彻底性、病人耐受度、意外拔管发生率等情况。结果:两组患者血气变化、鼻窦炎发生例数、置管合作程度方面无显著性差异,但在留管时间、口腔清洁程度、患者舒适度方面,经鼻气管插管组具有明显的优势。结论:对呼吸衰竭患者实施经鼻气管插管并机械通气的治疗措施,与经口气管插管比较,具有疗效肯定,能延长置管时间,改善口腔护理程度,有效防止口腔感染,使患者感觉舒适,防止意外拔管的优点,值得在临床上应用和推广。
Objective： To explore the clinical effect of nasotracheal or mouth intubati in patient with respiratory failure. Methods ： 31 cases with respiratory failure were divided randomly into group nasotracheal （14 cases） , group mouth intubati （17 cases）. Group na- sotracheal was treated with nasotracheal intubati , Group mouth intubati was treated with mouth intubation. The clinical features such as blood gas, placingtube time , nasosinusitis infection, the extent of mouth care, the tolerance of patients and incidence rate of unexpect- ed extubate were observed and compared in the two groups. Results： There were no significantly of the change of blood gas before and after the treatment between two group~ . There were no significantly of the case of nasosinusitis but there were significantly of the tracheal cannula keeping time and the depuratory of mouth cavity. Conclusion：Nasotracheal intubati on is definitely valuable for patient with respiratory failure. It is efficient and safe, and worthy of being recommended for use in clinical practice .
