基质金属蛋白酶-9及其抑制物-1在青年特发性气胸肺组织中的表达及意义 被引量:1

摘要 目的探讨基质金属蛋白酶-9(matrix metalloproteinase-9,MMP-9)及其抑制物-1(tis—sueinhibitors of metalloproteinase.1,TIMP—1)在青年特发性气胸肺组织中的表达及临床意义。方法随机选取我科2009年1月至2010年5月治疗的30例青年特发性气胸患者肺大疱组织标本作为实验组,同期获取10例青年正常肺组织标本(因肺裂伤行局部楔形切除肺组织)作为对照组,采用EVG染色法和免疫组织化学SABC法分别观察肺弹力、胶原纤维结构变化及MMP-9与TIMP-1在两组肺组织中的表达状况。结果实验组中肺弹力、胶原纤维密度稀疏,正常结构伴有明显断裂、萎缩,局部肺泡壁中有纤维缺失;对照组肺组织纤维结构基本完整。MMP-9在实验组肺组织中阳性表达率为76.7%(23/30),在对照组中阳性表达率为30%(3/10);TIMP-1在实验组中阳性表达率为66.7%(20/30),在对照组中阳性表达率为10%(1/10)(P〈0.05)。与对照组相比EMMP-9及TIMP.1在实验组中表达明显增多、增强,以TIMP-1表达升高更为显著,且存在MMP-9/TIMP-1表达比例失衡。结论MMP-9、TIMP-1在局部肺组织中的表达增多、增强及其比值的失衡与肺大疱的形成以及青年特发性气胸的发生、发展有着密切的关系。 Objective To evaluate the expression and significance of matrix metalloproteinase-9 (MMP-9) and tissue inhibitors of metalloproteinase-1 ( TIMP-1 ) in lung tissues of idiopathic spontaneous pneumothorax (ISP) in youngs. Methods Thirty specimens of bullous lung tissue from ISP young patients as experimental group were randomly collected from January 2009 to May 2010 and ten specimens of nor- mal lung tissue as control group. The methods of EVG staining and immunohistochemical analysis were used to observe the elasticity, collagen fibers structure, and expression of MMP-9 and TIMP-1 in two differ- ent groups. Results In lung tissues of ISP group, the density of elasticity and collagen fibers was lower and scattered by EVG staining, and local fibers deficiency was accompanied by fibers fracture and atrophy. In the normal control group, the lung fiber tissue structures were integrated. The positive expression of MMP-9 in ISP lung tissues was 76.7% (23/30)and in normal lung tissue was 30% (3/10). The positive expression of TIMP-1 in experimental group was 66.7% (20/30)and in normal control group was 10% ( 1/ 10) (P 〈0.05). The expression of MMP-9 and TIMP-1 in experimental group was significantly increased than normal control group, and TIMP-1 increased more significantly, with unbalanced ratio of MMP-9/ TIMP-1. Conclusion MMP-9,TIMP-1 and the ratio imbalance of MMP-9/TIMP-1 may play an important role in the occurrence of ISP in youngs.
