鸦胆子油乳瘤内注射辅助化疗治疗局部晚期非小细胞肺癌疗效观察

Efficacy Observation of Brucea Javanica Oil Emulsion Intratumoral Injection Combined with Chemotherapy in the Treatment of Locally Advanced Non-Small Cell Lung Cancer

目的探讨鸦胆子油乳瘤内注射辅助化疗治疗不可手术切除局部晚期非小细胞肺癌(LANSCLC)的临床疗效。方法将568例不可切除LANSCLC患者随机分为治疗组和对照组,每组284例。治疗组采取鸦胆子油乳瘤内注射辅助多西他赛+顺铂化疗方案治疗,对照组单纯采取多西他赛+顺铂方案化疗。治疗后比较分析2组患者的疗效和毒副反应。结果治疗组和对照组总有效率分别为51.4%、41.2%,差异有统计学意义(P<0.05);治疗组治疗后Karnofsky评分改善情况优于对照组,差异有统计学意义(P<0.05)。治疗组和对照组的毒副反应均主要为骨髓抑制、恶心呕吐和静脉炎。结论鸦胆子油乳瘤内注射辅助多西他赛+顺铂化疗方案治疗不可切除LANSCLC可提高疗效,而不增加化疗的毒副反应。

Objective To explore the clinical efficacy of brucea javanica oil emulsion intratumoral injection combined with docetaxel plus cisplatin regimen chemotherapy in the treatment of nonresectable locally advanced non- small cell lung cancer （LANSCLC）. Methods The 568 cases of nonresectable LANSCLC were randomly divided into the treatment group and the control group, 284 cases in each group. The patients in the treatment group were treated with brucea javanica oil emulsion intratumoral injection combined with docetaxel plus cisplatin regimen chemotherapy, and the patients in the control group were treated with docetaxel plus cisplatin regimen chemotherapy alone. After the treatment, the efficacy and toxicities were evaluated respectively. Results The response rate was 51.4% in the treatment group, and was 41.2% in the control group, there was significant difference between the two groups （P 〈 0.05 ） ; after the treatment, the difference in the Karnofsky score improvement of the two groups was statistically significant （ P 〈 0.05 ）. The main toxicities were marrow suppression, nausea and vomiting, phlebitis. Conclusion For the nonresectable LANSCLC, the brucea javanica oil emulsion intratumoral injection combined with docetaxel plus cisplatin regimen chemotherapy can improve the efficacy of chemotherapy, and doesn＇ t increase the chemotherapy toxicities.
