腹腔镜下分离阻断子宫动脉在子宫切除术中应用的临床分析 被引量:2

摘要 目的:腹腔镜下先分离阻断子宫动脉后再行子宫切除术与未预先分离阻断子宫动脉的子宫切除的进行临床对比分析,探讨腹腔镜下分离阻断子宫动脉在子宫切除术中应用效果。方法:分析2011年7-12月实施腹腔镜下分离阻断子宫动脉再行子宫切除术15例患者的临床资料(观察组),并在同期腹腔镜下未分离阻断子宫动脉的子宫切除病例中筛选条件相似的15例作为对照组。观察组预先游离出了子宫动脉,均用可吸收钛夹阻断子宫动脉后再行子宫全切术,对照组未预先分离阻断子宫动脉子宫切除术。比较两组术中出血、手术时间、手术并发症、术后胃肠功能恢复时间、住院时间。结果:两组手术均在腹腔镜下顺利完成,无中转开腹。其中观察组15例均成功分离出子宫动脉。观察组术中出血明显少于对照组[(40.32±39.50)mlVS(83.26±48.73)ml],差异有统计学意义(P<0.05)。观察组手术时间与对照组手术时间为[(60.60±24.70)min,(60.90±26.40)min],差异无统计学意义(P>0.05)。结论:腹腔镜下先分离阻断子宫动脉后再行子宫切除术可明显减少术中出血,避免损伤输尿管,是目前公认的最安全的术式。 Objective:To discuss the effect of uterine artery block before laparoseopic hysterectomy.Methods:There were 15 patients in Observation group(blocking uterine arteries betbre resecting hysterectomy) and 15 patients in control group(resetting hysterectomy directly)from Jul 2011 to Dec 2011.The blood loss,operative time,surgical complications,postoperative recovery time of gastrointestinal function,hospital stay were compared between the two groups.Results:All patients accepted successful laparoscopic hysterectomy.Observation group blood loss was significantly less than the control group(40.32±39.50)ml VS (83.26±48.73)ml,P〈0.05.Observation of operative time and operative time were nosignificant differenc(60.60±24.70)min VS (60.90±26.40)min,P〈0.05.Conclusion:Uterine artery block before laparoscopic hysterectomy can obviously decrease blood loss,avoid injury ureter,is widely recognized as the safest surgical procedures.
