输尿管镜钬激光碎石治疗输尿管结石的临床分析 被引量:16

摘要 目的:探讨经输尿管镜钬激光碎石治疗输尿管结石的临床疗效。方法:2008年2月-2011年8月笔者所在科收治的输尿管结石患者268例,均采用经输尿管镜钬激光碎石取石术,并对其临床资料进行分析。结果:本组268例病例中,一次碎石成功率92.9%,其中上段输尿管结石碎石成功率88.6%,中段输尿管结石碎石成功率94.7%,下段输尿管结石碎石成功率95.2%;手术时间26~90min,平均43min,上段输尿管结石手术时间38~90min,平均52min,中段输尿管结石手术时间28~67min,平均39min;下段输尿管结石手术时间26~58min,平均38min;术中出血量26~120ml,平均46ml,无输血病例;术后住院天数1~5d,平均2.5d;并发症发生率6.7%(18/268),其中术中并发症发生率5.2%(14/268),术后并发症发生率1.5%(4/268),术后并发症4例均为输尿管狭窄,3例行狭窄段钬激光内切开术,1例行狭窄段切除端端吻合术。结论:经输尿管镜钬激光碎石治疗输尿管结石具有方法安全、疗效确切、损伤小、出血少、恢复快、住院时间短、术中术后并发症少等特点,是输尿管中、下段结石治疗的首选方法;对于治疗输尿管上段结石通过改变术中操作、提高手术技巧、使用管路封堵器等手段,同样能够提高输尿管上段结石一次碎石成功率。
