社区高血压人群血压分级和体质指数与胰岛素抵抗指数的相关关系

The Correlations Between Homeostasis Model Assessment Insulin Resistance Index(HOMA-IR)and Hypertension Classification and Body Mass Index(BMI)Among the Hypertension Patients in Community

目的分析在社区高血压人群中,血压分级和体质指数(BMI)与胰岛素抵抗指数(HOMA-IR)的相关关系。方法选取社区中原发性高血压的随访病人203例,进行病史收集、体检,并测定空腹血糖、空腹胰岛素,计算胰岛素抵抗指数后,分析BMI、血压分级等因素与胰岛素抵抗指数的相关性。结果 (1)血压分级、高血压病程长短、高血压家族史、BMI、腹围及动物脂肪摄入对HOMR-IR呈正相关关系。而年龄和运动习惯则与HOMA-IR呈负相关关系。(2)在正常体重人群中,血压分级对HOMA-IR无明显影响;而在超重及肥胖的高血压人群中,血压分级为3级者较1级者有更严重的胰岛素抵抗(P<0.05)。结论减轻体重,改善不良生活方式,降低血压水平,有益于改善高血压人群的胰岛素抵抗。

OBJECTIVE To analyse the correlations between homeostasis model assessment insulin resistance index（HOMA-IR） and hypertension classification and body mass index（BMI） among the hypertension patients in community.METHODS 203 hypertension patients in community were included in this study,besides the collection of medical history and physical examination,fasting plasma glucose（FPG）,fasting insulin（FIN）,triglyceride（TG） and the high density lipoprotein cholesterol（HDL-C） level were also tested to calculate the homeostasis model assessment insulin resistance index（HOMA-IR）.Analyse the relationship between different factors and HOMA-IR,especially hypertension classification and BMI.RESULTS（1）Both the family history of hypertension and hypertension duration,hypertension classification,BMI,waist circumference（WC）,intake of animal foodstuff have the positive correlation with HOMA-IR（P0.01）.While,age and amount of exercise have the negative correlations with HOMA-IR（P0.01）.（2）Among the hypertension patients with normol-weight,hypertension classification doesn＇t have the effect on HOMA-IR,But among the obese patients with hypertension,the Grade-3 hypertensives get more serious IR than the others（P0.05）.CONCLUSION For the hypertension patients,reducing weight,changing the unhealthy lifestyle and lowering blood pressure are beneficial to improving insulin resistance.
