地佐辛复合氟比洛芬酯用于术后静脉镇痛的临床观察

Clinical Observation of Patient-controlled Intravenous Analgesia with the Combination of Dezocine and Flurbiprofen

目的:观察地佐辛复合氟比洛芬酯用于术后静脉自控镇痛(PCIA)的效果和安全性。方法:90例ASAI-II级全麻下行腹部手术病人,在手术完毕后行静脉自控镇痛(PCIA),随机分为3组(每组30例):地佐辛组(D组)、氟比洛芬酯组(F组)、地佐辛复合氟比洛芬酯组(DF组)。在PCIA开始后的3、6、12、24、48h采用视觉模拟评分(VAS)观察镇痛效果、用Ramsay法观察镇静效果、病人静脉自控镇痛(PCIA)按压次数,及镇痛后不良反应情况;病人总体的满意度。结果:参与术后镇痛的3组病人都有较好的镇痛效果,PCIA的按压次数差异无统计学意义,但DF组的镇痛效果最好,VAS评分均小于D组和F组(P<0.05),D组Ramsay评分术后6、12h高于F组和DF组(P<0.05),DF组术后的总体满意度显著优于D组和F组(P<0.05),且不良反应更小。结论:地佐辛复合氟比洛芬酯行术后静脉自控镇痛(PCIA),可提供良好的镇痛效果,不良反应少,是安全可行的镇痛方法。

Objective： To observe the efficacy,safety and side effect of dezocine combined with flurbiprofen in patient-controlled intravenous analgesia（PCIA） Methods： Ninety ASA I-II patients undergoing abdominal surgery under general anesthesia were randomly divided into three groups（n=30,each）： Dezocine（D）group;,Flurbiprofen（F）group;and Dezocine combined with Flurbiprofen（DF）group.After the patients received the PCIA,efficacy of postoperative and hypnosis were assessed by visual analog scale（VAS） and Ramsay method at 3,6,12,24,48h.The satisfaction of analgesia,the times of pushing button during the period of PCIA and side effects as well as patients satisfactory degree were recorded and compared. Results： The patients in three groups all had got the satisfactory efficacy of postoperative analgesia.There was no significant difference in pressing times of PCIA equipment.VAS was lower in DF group than in groups D and F（P0.05）.Ramsay sedation scale was higher in B group than in groups F and DF at 6 and 12h（P0.05）,DF groups of patients postoperative overall satisfaction is significantly better than D group and group F（P0.05）.Conclusion： Dezocine combined with Flurbiprofen in PCIA can provide satisfactory Postoperative analgesia and can effectively avoid the adverse effects.
