摘要
目的观察慢性阻塞性肺疾病急性加重期和稳定期患者血浆内皮素和降钙素基因相关肽水平的变化,探讨ET-1和CGRP在COPD发病机制中的作用。方法选择在山西医科大学第一临床医院呼吸内科2009年9月~2010年5月住院治疗的AECOPD患者30例,为急性加重期组,病情缓解后作为COPD缓解期组,同期健康体检者20例作为对照组。采用ELISA法测定3组血浆ET-1和CGRP的水平。结果急性加重期组分别与COPD缓解期组和对照组比较,ET-1水平显著升高(P<0.016,P<0.001)。AECOPD组与对照组比较,CGRP水平显著降低(P<0.001),COPD缓解期组与对照组比较,CGRP水平差异无统计学意义(P>0.05)。经Spearman秩相关分析发现AECOPD组ET-1与CGRP的水平呈负相关(r=-0.711,P<0.01)。结论 ET-1、CGRP参与了COPD发生、发展的病理生理过程,动态观察二者水平变化可作为判断COPD患者病情及疗效的一项指标。
Objective To measure the level of ET-1 and CGRP in patients with chronic obstructive pulmonary disease(COPD)and therefore to investigate the pathogenesy of ET-1 and CGRP in COPD.Methods Selected 30 patients with acute exacerbations of chronic obstructive pulmonary disease(AECOPD).When their illness alleviated,they were enrolled in COPD paracmasis group.20 healthy individuals were enrolled in control group.The plasma levels of ET-1 and CGRP were measured by ELISA method.Results The levels of ET-1 were significantly higher in AECOPD group than in COPD paracmasis group and in control group(P 0.016,P 0.001).The level of CGRP in AECOPD group was significantly lower than that in control group(P 0.001),but there was no significantly differences in COPD paracmasis group and control group(P 0.05).The Spearman rank correlation analysis that the level of ET-1 was negatively correlated with CGRP in AECOPD group(r=-0.711,P 0.01).Conclusion It is suggested that concentrations of plasma’s ET-1 and CGRP were possibly involved in pathophysiological process in the occurrence and development of COPD.Dynamic observation of the two levels in patients with COPD disease that can be used as an important indicator of the efficacy.
