超声斑点追踪技术用于老年高血压左心室纵向收缩功能的评价

摘要 目的探讨超声斑点追踪技术(STI)评价老年高血压(EH)左心室纵向收缩功能的价值。方法在左室射血分数(LVEF)正常的患者中,选择左室正常构型(LVN)的老年高血压患者21例(LVN组),左室重构(LVR)患者20例(LVR组),获取标准心尖两腔、三腔、四腔切面,测定左室各节段心内膜下纵向收缩期峰值应变,并计算3个切面的平均峰值应变和18个节段的平均应变,比较两组的差异。结果 LVR组与LVN组相比,3个长轴切面的平均峰值应变和18个节段的平均应变均显著降低(P均<0.01)。结论超声斑点追踪技术在评价老年高血压患者左心室发生结构改变时的整体和局部收缩功能具有重要的价值。 Objective To explore the value of the speckle tracking imaging(STI)in assessing the left ventricular longitudinal systolic function in elder patients with hypertension(EH).Methods The study population enrolled 41 elder patients of the normal Left ventricular ejection fraction(LVEF) with hypertension including 21 cases with left ventricular normal geometry(LVN) and 20 cases with left ventricular remodeling(LVR).Four-chamber,three-chamber and two-chamber apical views were obtained to determine the indexes including: three views of the peak systolic longitudinal strain of the subendocardial myocardium of LV segments,and mean peak strain of mean peak strain of 18 segments.Results Compared with LVR group and LVN group,the average peak systolic longitudinal strains of the subendocardial myocardium of each segment in three axis views and mean peak strain of 18 segments were decreased significantly(all P0.01).Conclusion It is important value in the speckle tracking imaging(STI) can assess left ventricular systolic function in the elder patients with hypertension.
