摘要
目的:比较术前静脉注射氟比洛芬酯注射液(氟比洛芬酯)对丙泊酚复合芬太尼全麻下行人工流产手术后下腹部疼痛的影响。方法:选择门诊自愿接受丙泊酚复合芬太尼全麻下行无痛人工流产的患者200例,根据不同的用药方法随机分为两组,每组100例。氟比洛芬酯组(F组)术前经静脉注射氟比洛芬酯50mg,然后缓慢静脉注射芬太尼1μg/kg,丙泊芬(1~2)mg/kg,待患者入睡后行吸宫术;对照组(C组)除未用氟比洛芬酯外,其余用药与F组相同。观测患者术后2、4、8、12、24h的视觉模拟评分,手术时间、镇痛满意度评分,并记录丙泊酚用量,术后出血量及恶心、呕吐等不良反应的发生率。结果:F组术后2h、4h、8h的VAS评分显著低于C组(P<0.05),而术后12h和24h的VAS评分比较差异无统计学意义(P>0.05);F组镇痛满意度评分明显高于C组(P<0.05),而F组丙泊酚用量明显少于C组(P<0.05);两组患者手术时间、24h出血量以及恶心、呕吐等不良反应的发生率的比较差异无显著性意义(P>0.05)。结论:术前静脉注射氟比洛芬酯能明显减少丙泊酚复合芬太尼全麻下行人工流产手术后下腹部疼痛,并能减少术中丙泊酚的用量。
Objective:To compare the effect of preoperative intravenous flurbiprofen axeyil on the lower abdominal pain in painless artificial abortion patients.Methods:200 patients undergoing painless artificial abortion in general anesthesia of propofol combined with fentanyl were randomly divided into two groups with 100 cases each:Flurbiprofen axeyil group(F) and Control group(C).Both groups were slowly intravenously injected fentanyl 1μg/kg,propofol(1~2)mg/kg.Besides,the patients in group F were intravenously injected with SOmg flurbiprofen axeyil.The visual analog scale(VAS),operation time.patient' s satisfaction,consumption of propofol and incidence of side effects were recorded during the period of postoperative 24 hours.Results:The VAS of group C at 8h after operation was significantly higher than mat of group F(P0.05),which became similar 12h later(P0.05).The patient' s satisfactions in group C were also significantly higher man those in group F(P0.05).There showed statistical differences in the consumption of propofol between two groups. The volume of hemorrage and the incidences of nausea,vomiting,itching.somnolence in group F and C showed no statistical differences. Conclusion:Preoperative intravenous flurbiprofen axeyil has a better effect on the lower abdominal pain and can reduce propofol requirements in painless artificial abortion patients.
