摘要
目的:探讨抗体捡测与彩色多普勒监测对结核性脑膜炎早期诊断价值。方法:60例结核性脑膜炎患者(观察组)和正常健康体检患者60例(对照组)都采用IgM、IgG抗体快速诊断和经颅多普勒(TCD)监测。结果:观察组的脑脊液IgM、IgG抗体检测阳性率都明显高于对照组(P均<0.05)。对照组:Vs、Vd、Vm、PI在大脑前动脉分别为(62.3±9.6),(94.5±14.2),(48.0±7.2),(0.75±0.14)。大脑后动脉分别为(62.8±9.5),(94.5±14.2),(47.9±7.2),(0.76±0.18)。观察组:Vs、Vd、Vm、PI在大脑前动脉分别为(115.2±12.5),(143.8±52.2),(82.3±12.5),(0.65±0.20)。大脑后动脉分别为(60.8±10.9),(97.5±14.2),(48.5±6.5),(0.75±0.18)。观察组在前动脉的各指标值与对照组相比都有明显差异(P均<0.05),而在后动脉中各指标值与对照组相比无明显差异(P均>0.05)。结论:抗体检测与彩色多普勒监测对结核性脑膜炎都有很高的早期快速诊断价值,两者结合可取长补短,使诊断结果更可靠。
Objective:To explore value of antibody test and color Doppler examination in early diagnosis of tubercnlous meningitis. Methods:Sixty patients with tuberculous meningitis(Observation Group) and 60 normal persons under healthy examination(Control Group) were put under IgM and IgG antibody rapid diagnosis and transcranial Doppler(TCD) examination.Results:Observation Group showed a significantly higher positive rate than Control Group(average P0.05) in both cerebrospinal fluid IgM and IgG antibody tests.For Control Group,Vs,Vd,Vm.PI for arteria cerebri anterior were(62.3±9.6),(94.5±14.2),(48.0±7.2),(0.75±0.14);Vs.Vd,Vm,PI for arteria cerebri posterior were(62.8±9.5),(94.5±14.2),(47.9±7.2),(0.76±0.18).On the other hand,for Observation Group Vs,Vd,Vm,PI for arteria cerebri anterior were(115.2±12.5),(143.8±52.2).(82.3±12.5),(0.65±0.20);Vs.Vd.Vm,PI for arteria cerebri posterior were (60.8±10.9),(97.5±14.2).(48.5±6.5),(0.75±0.18).Observation Group showed a significant difference with Control Group in various indexes of arteria cerebri anterior(average P0.05) and insignificant difference in arteria cerebri posterior(average P0.05).Conclusion: Both antibody test and color Dopplier examination are of high rapid diagnosis value for tuberculous meningitis.Both can be combined to exert both advantages and avoid disadvantages for a more reliable result of diagnosis.
