摘要
目的 提高对颅内混合瘤的CT、MRI表现与病理相关知识的认识。方法 12例经CT、MRI检查及手术光镜诊断 ,其中 5例作透射电镜肿瘤超微结构和免疫组化染色检查肿瘤相关抗原 (GFAP、Vimentin、S 10 0、FVⅢ因子 )。结果 脑膜瘤和胶质母细胞瘤 5例 ,脑膜瘤和垂体瘤 2例 ,胶质母细胞瘤和脑膜瘤、垂体瘤和脑膜瘤、胶质母细胞瘤和血管母细胞瘤、胶质母细胞瘤和血管内皮瘤、胆脂瘤和脑膜黑色素瘤各 1例。胶质母细胞瘤GFAP强阳性 ,S 10 0弱阳性 ;脑膜瘤Vimentin弱阳性 ,血管母细胞瘤和血管内皮瘤强阳性。结论 CT、MRI应结合临床上病程短 ,发展快等特点诊断本肿瘤。
Purpose To evaluate the knowledge of CT,MRI features and pathology in intracranial mixed tumor. Methods 17 cases were studied with CT,MRI and histopathology.Ultrastructure and tumor antigens (GFAP,Vimentin,S 100 and Factor FVⅢ) were observed by transmission electron microscope and immunoh istochemistry in 5. Results Operative pathological diagnose as follows:mixed tumor of meningioma with glioblastoma in 5,meningioma with pituitary tumor in 2,glioblastoma with meningioma,pituitary tumor with meningioma,glioblastoma with angioblastoma,glioblastoma with angio endothelioma,cholesteatoma with melanoma in 1,respectively.Strong positive of GFAP and weak positive of S 100 were showed in glioblastoma,weak positive of Vimentin in meningioma,strong positive of Vimentin in angioblastoma and angioendothelioma. Conclusion The diagnosis should be made when CT and MRI were combined with clinical features of short disease history and fast growth. [
