腹腔镜手术治疗胃间质瘤51例

目的：评价胃间质瘤的临床诊断价值及腹腔镜手术治疗的可行性与安全性。方法：回顾性分析2006年8月~2009年12月52例超声内镜拟诊胃间质瘤的临床诊断与治疗资料。结果：经腹腔镜手术胃间质瘤切除成功46例,成功率90.2%;术后病理组织学及免疫组织化学检查明确为间质瘤35例,超声内镜术前诊断准确率达68.63%。51例患者中平滑肌瘤6例,异位胰腺2例;炎性纤维息肉1例;神经鞘瘤2例;粘液细胞恶性肿瘤2例;炎症性肌纤维母细胞瘤1例;胃壁间血管球瘤1例。结论：超声内镜可以作为胃间质瘤临床拟诊的重要常规检查方法,腹腔镜下胃间质瘤切除手术治疗胃间质肿瘤是安全可行的。

Objective：To evaluate the clinical diagnosis of gastric stromal tumors and the safety and feasibility with Laparoscopic wedge resection.Method：The clinical data of diagnosis and treatment were retrospectively analyzed in 52 cases diagnosed as gastric stromal tumors by endoscopic ultrasonography.Results： 46 cases of laparoscopic surgical success rate 90.2%.35 cases were pathologically confirmed as gastric stromal tumors postoperatively,6 cases leiomyoma,2 cases ectopic pancreas,1case inflammatory fibropolypus,2 case neurilemmoma,2 case Krukenberg tumor,1 case inflammatory myofibroblastoma,1 case gastric wall glomangioma,Conclusion：Endoscopic ultrasonography can be selected as a main method for clinical diagnosis of gastric stromal tumors and laparoscopic resection was safe and feasible.
