腹腔感染对机体器官结构和功能的影响——一种多器官功能障碍综合征的动物模型 被引量:11

摘要 目的 :观察腹腔感染对兔多个器官系统的功能和形态结构的影响。 方法 :将 2 0只成熟期健康新西兰白兔随机分成感染组和对照组。感染组动物行盲肠结扎加穿孔 (CL P) ,对照组行盲肠探查术。两组动物均作颈内静脉置管术 ,输液维持 ,观察 1周。动态观察直肠温度、动脉血气、血清生化和尿中 3-甲基组氨酸排出量 ,记录动物死亡情况。在动物死亡或处死时抽血作细菌培养 ,取心、肺、肝、肾、小肠和脾等主要器官作光镜病理检查。 结果 :感染组动物在 CL P后直肠温度升高 ,血气分析异常 (Pa O2 下降 ,Pa CO2 升高和 p H值下降 ) ,血清生化异常 ,尿中 3-甲基组氨酸排出增多 ,血中细菌培养阳性率为 80 % ,器官病理改变显著 ,1周内动物均死亡 ,平均生存时间为 (84.1±39.0 ) h,与对照组比较有显著差异。 结论 :通过盲肠结扎加穿孔方法造成腹腔感染后 ,动物出现明显的器官功能和形态变化 ,具有多器官功能障碍综合征的特征。 Objectives:The study was designed to observe the effect of intra abdominal infection(IAI) on the function and structure of organs and systems in rabbits. Methods:Twenty animals were randomly divided into two groups,one received cecal ligation plus puncture(CLP) inducing IAI and another received sham operation.Rectal temperature,blood gas,serum biochemical indexes,urine excretion of 3 methylhistidine (3 MH) were measured dynamically.The animals were maintained with infusion for one week and the death was recorded.Blood bacteria and pathological changes of the heart,lung,liver,kidney,small intestine and the spleen were examined at death or when killed. Results:Compared with control group,the temperature was increased in IAI group animals,the abnormalities of blood gas and serum biochemical indexes occurred,the urine excretion of 3 MH was risen,and the pathological changes of organs were remarkable.The incidence rate of bacteremia was 80% and the survival time was (84.1±39.0) hours in IAI group. Conclusions:The functional and pathological changes in organs and systems caused by CLP induced IAI are obvious and characteristic of multiple organ dysfunction syndrome.
