闭合性肾损伤的螺旋CT诊断及临床应用价值

The diagnose value clinical value of spiral CT in closed renal trauma

目的通过研究各类型闭合性肾损伤在螺旋CT扫描中的表现,探讨其在临床诊疗中的应用价值。方法对经临床证实的55例闭合性肾损伤患者进行螺旋CT扫描,回顾性分析其CT表现和临床资料。结果肾挫伤11例,单纯肾包膜下血肿14例,肾挫裂伤22例,肾撕裂伤和尿外渗5例,肾粉碎伤1例,肾蒂伤2例,合并肾外损伤有肝脾损伤、肺挫伤、血气胸、肠破裂、骨折等。手术探查16例,其中肾切除4例。结论螺旋CT扫描及三维重建技术能迅速、准确地判断闭合性肾损伤范围、程度、有无合并肾外损伤等,对伤情分类作出正确评估,为临床制定诊疗方案提供依据。

Objective To study the various types of closed renal trauma in the performance of spiral CT scan,should be explored in clinical diagnosis and treatment of the application value.Methods Fifty-five cases of closed renal trauma that had been clinically confirmed were examined by spiral CT and were analyzed retrospectively with their CT findings and clinical data.Results Renal contusion in 11 cases,only intra-capsula hematoma in 14 cases,bruise rhegma in 22 cases,avulsion an external fistula of kidney in 5 cases,fragmentation in 1 case,renal pedicle injury in 2 cases,while there were rupture of hepato-spleen,lung contusion,pneumothorax,bowel rupture,bone fracture and so on.In 16 cases treated by surgical intervention,the kidney was resected in 4 cases.Conclusion Spiral CT scan and 3D image of CT scan is the rapid and effective detection with closed renal trauma.It is very important about the scope of closed renal trauma,extent,injury of other organs,etc.There are accurate for evaluating severity.
