经颞下梭状回入路选择性海马杏仁核切除术治疗颞叶内侧癫痫

摘要 手术治疗颞叶内侧癫痫,传统的前颞叶切除术疗效确切,但在切除局限于内侧颞叶的致痫灶的同时,过度去除了外侧颞叶新皮质,增加了失语、视野缺损和记忆障碍等神经功能缺失的发生几率。因此,Wieser和Yasargil等于1980年代创用了经外侧裂选择性杏仁核海马切除术,控制癫痫疗效满意,同时完整保留了颞叶新皮质,但手术必须在解剖外侧裂后于大脑中动脉分支间的狭小空间内操作,
