后腹腔镜输尿管中段切开取石术27例临床研究

Clinical investigation of retroperitoneal laparoscopic ureterolithotomy for middle ureteral calculi in 27 cases

目的研究腹腔镜治疗输尿管中段结石的手术疗效和临床价值。方法对笔者所在医院2009年6月～2011年3月收治的27例腹腔镜治疗输尿管中段结石患者资料进行分析总结。结果 27例均取石成功,无中转开放手术,手术时间60～135min,平均(95±13)min,术中失血<50mL,术后留院观察7～9d,无漏尿、感染,随访6～12个月,无输尿管狭窄及结石复发。结论后腹腔镜手术治疗输尿管中段结石创伤小、恢复快、安全有效,可基本上替代开放手术。

Objective To investigate the surgery curative effect and analyze the clinical significance of retroperitoneal laparoscopy ureterolithotomy for middle ureteral calculi. Methods Collection and analyze the clinical history of 27 cases of retroperitoneal laparoscopy ureterolithotomy for middle ureteral calculi between June 2009 to March 2011. Results Successful calculi removement for 27 cases and no conversion to open surgery. Duration of surgery is from 60min to 135 min,（95 ＋ 13）min in average. And blooding is less than 50 mL. There is no urine leakage in 7 to 10 days remain-in-hospital observation,no secondary ureterostenosis or no relapse of calculi in 6 to 12 months follow-up visit. Conclusion Therapy of middle ureteral calculi through retroperitoneal laparoscopy ureterolithotomy is a small wound, safe and reliable,short-term recovery and effective clinical method,and could substitute for open surgery in general.
