血清降钙素在呼吸道感染性疾病中的临床价值 被引量:9

摘要 目的探讨降钙素原(procalcitonin,PCT)在呼吸道感染性疾病诊断中的诊断及鉴别诊断价值。方法2008年6月~2011年6月入住我院呼吸内科住院部、急诊科的呼吸道感染性疾病患者78例,检测血清中PCT、C反应蛋白fCresponseprotein,CRP)、外周血白细胞计数(WBC)。结果与病毒感染组相比,细菌感染组患者血中PCT水平明显升高(P〈0.01),CRP、WBC的水平亦有明显升高(P〈0.01,P〈0.05);细菌感染组PCT、CRP、WBC的阳性率明显高于病毒组(P〈O.01),但三个指标中PCT阳性率明显高于CRP、WBC;PCT在三个指标中的特异度、敏感度、阴性预测值、阳性预测值、准确性均为最高。结论PCT对呼吸道感染性疾病的临床应用价值优于CRP以及外周血白细胞计数,可作为诊断呼吸道感染性疾病的参考指标。 Objective To discuss the application value of the detection of serum procalcitonim (PCT) in the diagnosis of respiratory tract infection. Methods Seventy-eight samples were collected from patients with respiratory tract infection in respiratory department and emergency department from Jun. 2008 to Jun. 2011, PCT and CRP were measured, and white blood cells were counted also. Results The differences of PCT, CRP and WBC between the bacteria infection group and the virus infection group had significant difference (P 〈0. 01 or P 〈0.05). The positive rates of PCT in the bacteria infection group, was superior to serum level of CRP and WBC. PCT had highest specifieity, negative predictive value, positive predictive value,and accuracy amond three index. Conclusion The serum level of PCT is superior to serum level of CRP and WBC in application value for diagnosis of respiratory tract infection.
