摘要
目的探讨经鼻入路手术治疗复杂眶壁骨折的效果。方法 2001—2010年,我院收治的复杂眶壁骨折病人21例,其中筛窦型13例,上颌窦型3例,筛窦和上颌窦复合型5例,根据病人眶壁骨折类型和创伤部位的面积,采用X线胶片片基自行研制支架,实施经鼻入路置入手术,观察病人术后复视、眼球内陷及颜面部外形情况。结果术后16例复视消失;5例轻度复视,其中3例在术后3个月后恢复正常,另2例有轻度复视。19例眼球内陷术后即完全矫正,2例术后轻度内陷。颌面外形正常或基本正常。结论采用自制支架经鼻入路手术治疗复杂眶壁骨折,组织损伤小、并发症少、疗效满意,值得推广。
Objective To investigate the effects of transnasal approach operations in the treatment of complicated orbital fractures（COF）. Methods From 2001 to 2010,21 patients with COF were admitted,of whom,13 had fractures of ethmoid sinuses,three of maxillary antrum,and five of complex type of ethmoid sinuses and maxillary antrum.A self-made stand using X-ray film,based on the type of fractures and wound area,was inserted through transnasal approach.Postoperative diplopia,enophthalmos,and facial surface were observed. Results Postoperatively,diplopia disappeared in 16 patients.Five had mild diplopia,among them,three recovered three months after surgery,and two had mild diplopia.Enophthalmus was completely corrected in 19 patients,while mild enophthalmus remained in the other two.Their occlusalf surface was normal or basically normal. Conclusion Transnasal application of self-made stands for complicated orbital fractures has advantages of little tissue demage,few complications,and satisfactory therapeutic results.
